Records Request

Families, Employers and Companies:

To verify graduation or attendance, a signed release needs to be provided authorizing the organization to obtain the information.

Please send your request to the student's previously attended school. If the school is closed, you may send the request to the contact information below.

If you do not have a request form, please fill out our Educational Records Request Form. Please send completed form and a copy of your photo ID to:
