Incident Reporting
Per OSHA, an incident can be described as an unanticipated event that does not cause significant losses. It can be an event that results in unfavorable outcomes such as mild illness or injury, property damage, or a “near miss” with a profound health effect, or a decrease in production.
Employees can submit an incident report through Frontline Central found under the "My Apps Dashboard"

Workers Compensation Claim
If your workplace incident or injury requires care outside of the school district please take the following steps as soon as possible:
- Notify:
- Notify your supervisor of the incident/injury
- Complete the employee incident report (found on Frontline Central - see above)
- Notify the RSD Workers Comp Coordinator to obtain a SIF-2 form and claim number
- Seek Medical Care:
- Initial medical treatment can occur at any emergency room or healthcare location of the employee's choice.
- For your second visit and beyond, you will need to see a provider within L&Is provider network.
