Employee Benefits Out Reach Events

In an effort to assist District employees with the transition to SEBB, the payroll/benefits team will be hosting out reach events.  These events will be constructive events with personal/private assistance with enrollment on the new WA State health plans offered through SEBB that become effective January 1, 2020 for those eligible. Including basic set up of your SEBB online my account, assistance with interactive enrollment tools and upload of documents providing proof of dependent eligibility.

Please note that Kaiser, Premera, UMP representatives, or other vendors DO NOT attend these events.  If you have questions about these products please contact the provider directly at Kaiser | SEBB, or Premera | SEBB, or UMP | SEBB, and/or other optional benefits.

Please feel free to bring additional support and if you have need of an interpreter, please let us know in advance so one can be ready to assist you.

Forms Needed For Dependent Verification


  • Most recent federal tax return, or
  • Marriage certificate, and
    • Proof of common residence
    • Proof of financial interdependency

Children up to age 26

  • Most recent federal tax return, or
  • Birth Certificate

State-registered domestic partner

  • Certificate of state-registered domestic partnership, and
    • Proof of common residence, or
    • Proof of financial interdependency

Extended dependent

  • Extended dependent certification form, and
  • Court order
    • Serves as dependent verification
    • Dependent must reside with subscriber

Disabled dependent

  • Certification of a child with a disability form, and
    • Disability renders child incapable of self-sustaining employment
    • Dependency upon subscriber for support
    • Disability must have occurred before the age of 26
  • Most recent year's federal tax return, or
  • Birth certificate
