Referral and Appeal Forms
Any community member may refer a child to participate in the identification process. However, the child’s parents or guardians must formally allow their child to participate in the additional testing that is part of the process. If identified, the parents or guardians must also request and select placement into Highly Capable programs.

Referral deadline - February 1, 2019 with services to begin in the 19-20 school year should your student qualify.
If your student is new to district (all Kindergarteners and any student that enrolled on or after January 1, 2018 and did not refer last year), if you submit your referral by December 1, 2018 AND attend a testing date no later than January 14, 2019, services will begin February 25, 2019 should your student qualify.
Parents/legal guardians have the right to appeal the district’s Highly Capable decision. Individuals appealing the selection committee’s decision must submit a completed appeals form or letter requesting a review of selection/placement decision. The written request must be provided within two weeks of the parent/legal guardian’s receipt of the decision, include reasons for the appeal and provide additional evidence of significantly advanced cognitive or academic levels and/or outstanding intellectual or academic abilities.
