Immunization Information

  • The State of Washington requires immunizations before a child may begin school.  Students must meet minimum intervals and ages to be in compliance with the requirements; check spacing days between shots.  See your doctor with all school forms if not on track with spacing.  The School nurse monitors immunization compliance.  See web page link below. 

    • Immunizations must be listed on a Washington Certificate of Immunization Status that has been signed by the student’s parent or guardian.
    • A photocopy of a Washington Certificate of Immunization Status from a student’s previous school is acceptable.
    • If you plan to exempt your child from one or more immunizations, a Certificate of Exemption form is available in the school office and on the RSD website.  A health care provider signature is required.  Note:  Don’t forget to select which specific shot(s) are being exempt.

    How can I access my or my child's immunization records?

    You may obtain your own or your child's immunization records from Child Profile. However, the Child Profile Immunization Registry does not contain complete immunization records for all people. It's a good idea to check with your health care provider first to see if they can get you the information you need.

    If you can't get a complete immunization history from your provider, contact Child Profile to find out if the Immunization Registry contains any immunizations for you or your child. Call Child Profile at 206-205-4141 or 1-800-325-5599. Or, fill out the Authorization to Release form (PDF, 100KB). A Spanish Authorization to Release form (PDF, 27KB) is also available.     When you call, you'll be asked to provide identifying information, such as name, date of birth, and address. At that time, Child Profile will tell you whether or not a record exists in the system, but no other information will be released over the phone. This is to protect the privacy and confidentiality of your information. Once the signed Authorization to Release form is received, Child Profile will mail or fax the immunization record to you within three to five days.
